Please Join us remotely this year for the DQOLC Annual Hospice and Palliative Care Conference on April 22 2-5pm via Zoom.

In years past this conference was combined with our Annual Excellence Awards Dinner; however due to ongoing constraints related to the pandemic this year we have separated the conference and the dinner.
The conference will be held remotely as outlined below and the dinner is being planned for a later date in June with details to follow.

• 2:00 - 3:00pm  ‘What it takes to Direct a HPM Fellowship’, Carly Levy, M.D., Program Director, Nemours/CCHS/SKMC HPM Fellowship
• 3:00 - 4:00pm  ‘Orientation program for APC in Supportive and Palliative Care’, Amanda Zettle, APRN, CCHS SPC Program.
• 4:00 - 4:15pm BREAK
• 4:15 - 5:15pm  ‘Community ACP and Education’, Nicole Fullmer, Delaware Hospice ‘Honoring Choices’ Program.
